Camper Required Forms Packet
After registering for camp you will recieve a newsletter called The Cougar Quest Chronicle along with a packet of forms that are required to attend camp. You should receive these items within two weeks of registering. If you have registerd and have not recieved your Chronicle or forms packet please notify us via e-mail at cougarquest@wsu.edu.
Once you do receive the forms packet by mail, please return the forms to our office ASAP to maintain your first choice selection of workshops. Forms should be returned within 2 weeks of receipt.
The Forms Packet will include:
- Assumption of Risk
- Participant Health Form
- Emergeny Medical Release
- Image and Voice Consent
- Travel Plans
- Camper Code of Conduct
