Thank you for your commitment to enriching your child’s intellectual and social growth through participation in Cougar Quest.  Now that your child has had a chance to tell you about their Cougar Quest experience, we would appreciate your input as we plan for next year.  Your response is voluntary and you may leave any question blank, but completion is greatly appreciated.  Responses to the survey will be summarized and results will be reported to the program manager and director.  No names will be used in the reporting of results.
